We are seeking a reliable and motivated Rehabilitation Assistant to join our cohesive team within the
friendly community of Creston, BC.
Swan Valley Lodge Long Term Care facility embodies a resident-centered philosophy designed to
support residents’ choices, help people function at their highest level, and provide the best quality of life
possible. This facility is a 90 bed facility and includes six respite/convalescent beds and a 23 bed
dementia unit. Along with an Occupational Therapist and a Physiotherapist there are a total of two
Rehabilitation Assistants supporting the care of residents.
The Rehabilitation Assistant is a member of the multidisciplinary healthcare team in meeting the needs
of the residents. The Rehabilitation Assistant carries out rehabilitative treatment plans and programs
and assists therapists in monitoring and evaluating residents’ progression. In addition to supporting
therapy with the short stay and long term care clients there is a small amount of outreach involved in
this role.
Shifts are rotating and scheduled during the Day. We offer an exceptional employer-paid benefits

Required Qualifications: Completion of a recognized two-year Therapy Assistant or Rehabilitation
Assistant Program. Valid Driver’s License.
